Our New Life Behavior program is a ten-month program, and is often filled to capacity. Because of this, we have established the following list of requirements that must be met prior to being considered for admission: 1. We require that the man interested in the program write us a personal Introductory Letter giving a brief sketch of his current situation and why he wants to come to our facility. This letter should also include the following information: o Name, age, address and phone number o Occupation, present employer and work history o Marital status and number of dependents o How your family will be cared for while here o Type of addiction and number of years in it o Previous rehabilitation programs attended o Current court charges o Probation information o Complete criminal history o Medical problems requiring special attention
Mail completed letter to:
1. We will accept your phone call to discuss your status 5-7 days after you have mailed your letter. If you fax or e-mail your introductory letter, give us two days to review your letter then call the office. 2. All interview appointments occur on weekdays. No person will be accepted for an interview at night or on weekends except by previous arrangement. 3. The personal interview should in no way be interpreted as an automatic acceptance into Three Corners. Its purpose is to discern if the man is truly ready to receive help, and to determine how we can best meet his particular needs. 4. We require that a man be completely off drugs or alcohol for three days prior to the interview. We will absolutely not take men into our program that are intoxicated or under sedation. 5. We do not allow pharmaceutical drugs such as tranquilizers, sleeping pills and antidepressants. 6. Persons who require medical attention will not be accepted into our program as we are not prepared to meet special medical needs. 7. Each man is expected to bring toilet articles, towels, work clothes and casual dress clothes if possible. We do, however, provide these items to any individual in need of them. 8. When arriving for scheduled interview, be sure to bring your social security card and a picture ID, such as a driver’s license or a police ID.
Please include all probation information and pending court dates. The single most important factor to a man being admitted to Three Corners is his attitude. Is the applicant ready to radically change his lifestyle? He should describe in the letter his sincere desire for spiritual help. |
